Both Ethan Hawke and Josh Charles have doctor badges on that say Dr. Anderson(Ethan Hawkes's character in the Dead Poets society is called Todd Anderson) and Dr. Overstreet(Josh Charles' character in the film's name is Knox Overstreet) which I think is a fun little detail that Taylor added. I actually think this music video is going from the current state at the start where she has tattoos of post. It’s harley quin and the joker vibes. Like she copied his look because she was so obsessed with him. Then it flashes back to her meeting him.
