A few tips:
1) Unless the dealer has a magnifying glass, or it's the last bullet, he won't know what's actually in the gun. If he doesn't know what the current bullet is, he has a 50/50 shot at aiming at himself or you, regardless of what's in the gun. This is also why he wastes items a lot, and this also means he can't calculate the odds of lives to blanks.
2) Since it's your turn automatically at the beginning of the loadout, if there is a small number of bullets in the gun and you have items such as beer or handcuffs, you can prevent his turn entirely. For example, if there's two bullets in the gun, you can handcuff him and shoot both at him, you can use a magnifying glass to know for sure what both bullets are and act accordingly, or you can use a beer to lower the number of bullets left. This can also be good if the dealer has cigarettes to stop him from using them to heal himself.
3) In most cases statistically, it's actually better to shoot the opponent regardless of what's in the gun, except for a few niche situations. This is mainly because the dealer is an idiot, so for example, if there's a few live rounds left in the gun, as long as it's not the last bullet (since he'll automatically know what it is), he has a good chance of just shooting himself in the face.
The pills at the beginning of the game also activate Double or Nothing mode, which is basically an endless mode where various things are randomised, and you can keep on doubling your score as much as you want, as long as you don't die.
Edit: Remembered at the end of the video you said what's the best you can get score-wise. The initial score is randomly generated between 50k and 70k, but then Double or Nothing mode can let you increase that endlessly as long as you don't die.

26:04 the suspense built up here is insane.. him being in handcuffs, the music, the number of lives no longer showing, the opponent slicing the shotgun and last of all.. him asking "Are you ready?" All of that makes this game feel real and it's not only scary but insane. They really did a good job with this game! 🔥🔥
