Boy:No words ,just shy's away,smiles a little coyly....but .makes extra effort to hide the teeth protruding out almost 12 mm,not allowing him to keep lips closed.
Father: sab isko class mein tease karte hain
( Translation: everyone teases him in class) .....one dentist recommended your name......
Always an honor to get referrals from colleagues who know me from these groups and haven't ever met some of them...puts added pressure to deliver.
Other side of story:
What's getting more common is a chairside instant diagnosis on same appointment ,without proper documentation and an attempt to get those teeth back immediately,by hiding behind an expensive ๐ฐ ๐ฐ brand name ,by promising patients ,we will do Daemonic/self ligation new hi-tech braces / plastics, that promise no pain and all the junk child can eat and all will be right with your kid ..
it's important to take a breather and study the face well ,than just the teeth,we all were taught that....
These cases are where it's good to take help of cephalometrics ,but only second to clinical photo documentation, and also do a post Dentofacial orthopedics ceph and I ask my centre to do superimpositions....only cases I subject a child for cephalometrics for guidance before and thru the treatment,personal choice , nothing against specialists , who do cephalometrics for each and every case,each one to his own self.
Learning functional orthopedics and Interceptive Orthodontics procedure, is every dentist duty,(but mostly the requests are to teach comprehensive Orthodontics for the lure of easy money)....it's only diligent ones who try to put an effort to learn these life changing therapies that ,we are blessed to offer.....
This boy was an introvert and just didn't know what a heart full smile is ,as he struggled hard with those jutting out teeth....
The conventional visualisation of treatment objective and then functional orthopedics with Twin block appliance,followed by a holding appliance after some bite ( not all) is jumped,followed by braces at last and then a long term of retention ,is what is the standard protocol.
Important is the confidence and change in the personality of the child in his growing years,the friendships he/she develops un-hindered,no matter if the treatment is not hundred percent perfect ,no matter we have some lower proclination as per ceph values ( but it shouldn't be uncontrolled loss though) .Don't treat to values....treat to a functional bite...
All is well, if soft tissue drape enables a personality change in these important years.
