👉What? It can be a quick 5-minute or in-depth assessment used to evaluate the status of a patient’s health.
👋 INTRO: Hand hygiene, identify patient, vitals, alert & orientation
🗣 HEAD
Mouth, Gums, Tongue: moisture + color
Pupils: PERRLA (pupils equal, round, and reactive to light & accommodation)
Neck Veins: flat = normal, distended = ⬆️ fluid
🫀 CARDIOVASCULAR
Auscultate apical pulse for 1 full minute. Note: rate, rhythm, regularity.
⚠️ Normal HR: 60-100 BPM
🫁 PULMONARY
Auscultate anterior/posterior lungs in an S-shaped pattern. Note: depth, rate, sounds
⚠️ Normal RR Rate: 12-20
⚠️ Normal O2 Sat: 95-100%
🚽 GI/GU
Inspect, Auscultate, Palpate. Note: bowel sounds, last void or BM
💪 Extremities
Assess Skin (clean, dry, intact)
Cap Refill: normal <3 sec
Bilateral Strength + Pulses
Edema: 0=none, +1 = 2 mm, +2 = 4 mm, +3 = 6 mm, +4 = 8 mm
♿︎ Equipment
Assess IV Sites for complication: phlebitis, extravasation, infiltration
Adaptive devices check (wheelchair, walker)

⚠️ NCLEX Tip: The first stool "meconium" is passed within 24-48 hrs after birth. It is thick, sticky, and black.
✅ YELLOW = common in breastfed babies
✅ DARK GREEN = normal as meconium cleared
✅ BROWN = common in formula-fed babies 🍼

IV Therapy Complications 💉🔥
Starting and maintaining an IV is not as easy as 🥧PIE🥧 because complications can happen.
🔥 Just remember PIE:
✨P✨ for Phlebitis: inflammation of a vein
✨I✨ for Infiltration: leak of IV fluids or meds into the surrounding tissue.
✨E✨ for Extravasation: leak of Vesicant Drugs (damaging) into the surrounding tissue.
⚠️Always assess the IV site and stop an infusion if you suspect an IV therapy complication has occurred.

👉What? There are 4 STAGES: Labor, Birth of Baby, Birth of Placenta, and Postpartum.
👉Remember the 3 PHASES in the 1st stage of labor with “LAT”: Latent, Active, and Transition phase.
🏥 Nursing Interventions
✔️ Vitals Q5-30 min depending on the situation
✔️ Continous FHR monitoring
✔️ Encourage voiding Q2 hrs
✔️ Assist with position changes
✔️ Pain management (pain meds, breathing techniques)
✔️ Prepare for neonate: preheat warmer, check O2 and suction, prepare resuscitation equipment and emergency meds
⚠️ NCLEX Tip: The more time spent in Stage 3 (birth of placenta) = the higher risk of infection/hemorrhage. Prepare to administer oxytocin to prevent hemorrhage.
