👉 Truth: I used to fail nursing exam questions that asked which vaccines were needed at each age. It used to be so hard for me to remember.

🔥 Here is an EASY way to remember in just 4 sentences:

🍼 Newborn: Hep B. B = BIRTH
🧸 2 Months & 6 Months: Be DR HIP
🪁 4 Months: DR HIP
🐥 12 Months: HI VAMP

There were times I wanted to quit nursing school. 💔🩺

🌼 I felt like I did not belong because:

1️⃣ I had mental health issues (depression, anxiety)
2️⃣ I was a terrible test-taker
3️⃣ I was told to switch career paths by others

🌼 Imposter syndrome can creep in and make you question if you belong. BUT YOU DO BELONG.

🌼 You are not here by accident. You've earned your place.

Please remember:
👉 If you got into nursing school, you PROVED you are smart enough to pass the program.
👉 If you finished nursing school, you PROVED that you can pass the NCLEX exam.

The fact you are worried about being good enough for your future patients just shows you are going to be an incredible nurse!

The late nights, long shifts, and endless studying will pay off. ❤️

Head-To-Toe Assessment Basics 🩺🫀

👉What? It can be a quick 5-minute or in-depth assessment used to evaluate the status of a patient’s health.

👋 INTRO: Hand hygiene, identify patient, vitals, alert & orientation

🗣 HEAD
Mouth, Gums, Tongue: moisture + color
Pupils: PERRLA (pupils equal, round, and reactive to light & accommodation)
Neck Veins: flat = normal, distended = ⬆️ fluid

🫀 CARDIOVASCULAR
Auscultate apical pulse for 1 full minute. Note: rate, rhythm, regularity.
⚠️ Normal HR: 60-100 BPM

🫁 PULMONARY
Auscultate anterior/posterior lungs in an S-shaped pattern. Note: depth, rate, sounds
⚠️ Normal RR Rate: 12-20
⚠️ Normal O2 Sat: 95-100%

🚽 GI/GU
Inspect, Auscultate, Palpate. Note: bowel sounds, last void or BM

💪 Extremities
Assess Skin (clean, dry, intact)
Cap Refill: normal <3 sec
Bilateral Strength + Pulses
Edema: 0=none, +1 = 2 mm, +2 = 4 mm, +3 = 6 mm, +4 = 8 mm

♿︎ Equipment
Assess IV Sites for complication: phlebitis, extravasation, infiltration
Adaptive devices check (wheelchair, walker)

Newborn Stool Colors 🍼🌈

⚠️ NCLEX Tip: The first stool "meconium" is passed within 24-48 hrs after birth. It is thick, sticky, and black.

✅ YELLOW = common in breastfed babies
✅ DARK GREEN = normal as meconium cleared
✅ BROWN = common in formula-fed babies 🍼
✅ BLACK = 1st stool "meconium"

❌ RED = bleeding 🩸
❌ ORANGE = malabsorption
❌ BRIGHT GREEN = infection or allergy
❌ BLACK = GI bleed (only after 1st week)
❌ WHITE / PALE = liver or gallbladder issue

👉 What? Blood products are used for anemia, trauma, surgery, or bleeding disorders.

⚠️ NCLEX Tip: Always verify the C's BEFORE administering blood: compatibility (via crossmatching), consent obtained, complete baseline vitals, confirm patient identity, and call for a 2nd nurse to sign off.

I was tested frequently on HF in nursing school. Memorize below 👇👇👇

🫀 LEFT = LUNGS 🫁
SX: respiratory wheezes, dyspnea, cyanosis, cough w/ blood-tinged sputum

🫀 RIGHT = REST of body 💦
SX: edema, ascites (water in abdomen), weight gain, liver & spleen enlarged

⚠️ NCLEX Tip: Digoxin is commonly prescribed with HF. Assess apical HR for 1 minute BEFORE admin and HOLD if apical pulse <60. Digoxin toxicity is >2 ng/mL.

IV Therapy Complications 💉🔥

Starting and maintaining an IV is not as easy as 🥧PIE🥧 because complications can happen.

🔥 Just remember PIE:

✨P✨ for Phlebitis: inflammation of a vein
✨I✨ for Infiltration: leak of IV fluids or meds into the surrounding tissue.
✨E✨ for Extravasation: leak of Vesicant Drugs (damaging) into the surrounding tissue.

⚠️Always assess the IV site and stop an infusion if you suspect an IV therapy complication has occurred.

👋 My name is Amanda and I have been a Peds RN for almost 3 years.

🌸 I used to live paycheck-to-paycheck in an apartment that had been infested while working as a medical receptionist. I also lost 2 babies previously, had a parent with cancer, AND had issues with depression, attention, and anxiety all WHILE attending nursing school.

Stages of Labor 🍼

👉What? There are 4 STAGES: Labor, Birth of Baby, Birth of Placenta, and Postpartum.

👉Remember the 3 PHASES in the 1st stage of labor with “LAT”: Latent, Active, and Transition phase.

🏥 Nursing Interventions
✔️ Vitals Q5-30 min depending on the situation
✔️ Continous FHR monitoring
✔️ Encourage voiding Q2 hrs
✔️ Assist with position changes
✔️ Pain management (pain meds, breathing techniques)
✔️ Prepare for neonate: preheat warmer, check O2 and suction, prepare resuscitation equipment and emergency meds

⚠️ NCLEX Tip: The more time spent in Stage 3 (birth of placenta) = the higher risk of infection/hemorrhage. Prepare to administer oxytocin to prevent hemorrhage.
