SPECT stands for single photon emission computed tomography. A radioactive tracer like technetium sestamibi is injected intravenously and nuclear scan images using a gamma camera obtained after 20 minutes. This will show the uptake of the tracer at various regions of the heart.
After that those who can walk on a treadmill will be connected with electrodes for monitoring ECG during the test. Treadmill slope and speed are typically increased every three minutes and maximal exercise without angina or ECG changes is achieved. The test is terminated earlier in case of significant angina, other major symptoms, or ECG changes. The ECG printouts are analysed by the supervising physician.
At maximal exercise, one more injection of radioactive tracer is given. One more nuclear scan is done after waiting about 20 minutes with the gamma camera. The rest images are compared with the exercise images by the nuclear medicine physician.

Here is a normal 12 lead ECG. 12 lead ECG consists of three standard limb leads (Leads I, II and III), three augmented limb leads (aVR, aVL, and aVF) and six chest leads (V1 to V6). All the twelve leads have recorded ECG of three cardiac cycles in this tracing.
There is a long lead II recording at the bottom of the tracing which is a rhythm strip enabling better assessment of the cardiac rhythm.
12 Leads can be acquired simultaneously and printed sequentially as in a 12 channel machine or can be acquired sequentially as in a single channel ECG machine. This particular tracing was recorded using a 12 channel ECG machine.

For medicos: Left bundle branch block (LBBB) induced cardiomyopathy is due to the mechanical dyssynchrony caused by the abnormal activation pattern in LBBB. Response to cardiac resynchronization therapy (CRT) is excellent in this group and may be called super responders. Left bundle branch pacing has shown promise an alternative to biventricular pacing in LBBB induced cardiomyopathy.

For medicos: The term โ€˜Windkessel effectโ€™ is used in the setting of large elastic arteries like aorta. The original term โ€˜Windkesselโ€™ means โ€˜air chamberโ€™ in German language. It was an air chamber used in fire engines in the 18th century, to maintain continuous delivery of water for fire-fighting. In the case of aorta, the aortic elasticity causes expansion and temporary storage of blood in systole so that more blood enters the aorta than what leaves it in systole. This buffer stock of blood is released during diastole, to maintain organ perfusion. The Windkessel effect makes aorta function as a โ€˜second pumpโ€™ in diastole. This diastolic flow is quite important in the case of coronary perfusion as most of the coronary perfusion is diastolic.
