I have figured out how to build a Nether portal underwater. You can make it as tall as you want, up to the maximum, but it has to be the standard 4 blocks wide. Step 1: Build the frame. As I just said, you can make it as tall as you want, but needs to be 4 blocks wide only. Build it around the Ocean Monument or under it.

EDIT: Actually was pretty promising, but it wont form naturally. BUT it apparently is possible to make a portal without a diamond pick. Yep its called casting, just need a bucket some water and lava and bam. You should check out youtube, and the group mindcrack and something called UHC (ultra hardcore).

I'm using the Better Nether mod, and the standard 3x2 portal from won't light, am I missing something there's nothing on the mod page about a different portal set up. Have you tried making it bigger?
