Generally just a safety and/or motivation thing. For the most part the top guys pacers are just running behind them. Leadville is a bit unique in that pacers are allowed to "mule" or carry stuff for the runner. Most races require the runner to carry all of their own nutrition/gear. I've run 100's both with and without pacers.
