Synths (human like - level 3) will "infiltrate" your settlements. Blame Railroad. You don't know who is a synth. Sometimes a resident will regognize a synth and kills it, and you have to resolve the situation as a Minuteman. Preston or radio says a settlement is under attack, but it's only a resident-synth kill.

My largest settlement Starlight Drive with twenty settlers has all the beds, food, water, and defense it needs and more. Randomly there is a decrease in happiness rating for no apparent reason so I am wondering if this random decrease in happiness is due to synth infiltration. I checked the settlement with VATS scanning every settler.

The Institute may send older synth models to your settlement much like a Raider attack. In the attack will those settlers, who are actually synths, switch sides and fight you. It is however not as tough of a fight as it may sound. I found the attacks to be weaker than an actual Raider attack in comparison.

Assign him to a supply line, follow him until he's out of range of the settlement, then kill him. Send him to a vacant settlement and dispose of him there. I usually just keep myself from getting synths to start. They only show up if you have more than 5 settlers in a settlement.

Synth settlers are 100% normal settlers, they have normal happiness, and behave as "normal" as normal settlers do, including disobeying orders, standing around in weird groups, failing to have bed shelter detect properly, and getting stuck on rooftops. Regular human things, and they are safe to have.
