I think the KEY of the match was the break Sinner won in the second set. There, in that moment, he changes his strikes. He realises he can break Medvedev again and becomes much more confident. Even though he loses that set he knows exactly what to do for the rest of the match.
Tennis is a matter of head and nerves.

Also what a gruelling run for Medvedev though, 24 hours on the court and he still had the reserves to take Sinner to five sets with his aggressive play.
