In all fairness to House, at 1:00, explaining the twinning situation to a couple who are exhausted and have nowhere near enough medical knowledge to understand whats going on, plus dealing with an incredibly emotional mum is incredibly difficult yet he actually dealt with it incredibly well.

The tapeworm scene is from an episode in which the patient was unable to feel pain but had learned to fake feeling pain. That's why he says she is faking it.
