There's a law of beyonder characteristics convergence. The core law of the novel in play.
It's stating that. Beyonder attract beyonders. But there's more into that(spoiler free because it's explain in the early of novel).
Beyonder of the same pathway attract each other. Not only that, their neighboring pathway also attract neighboring pathways. And there's also a beyonders of another pathway attract to you because you associated with one of their neighboring(or the same) pathways.
No matter how great you can hide. You're destinied to attract the danger both upon yourself and your surroundings.

These voices aren't universal between pathways, the "ravings" are the result of a beyonder possessing a uniqueness choosing to broadcast themselves to all lower beyonders in their pathway. For Old Neil's case this was especially bad as the highest authority of his pathway (the Hidden Sage) is a notorious menace that instils people with knowledge harmful to them.
Beyonders in pathways of which there are no gods, uniqueness bearers, or simply the presence a benevolent god/uniqueness bearer do not hear these ravings whatsoever.
