After meeting a little girl named Sherry and rescuing her from the giant monster with eye ball on it's shoulder, Claire Redfield finds herself being chased once again by the same creature. She tells Sherry to continue running ahead of her as the monster is gets closer. They quickly scurry into a cargo elevator to escape the monster. Claire slams on the button to shut the doors and descend, but just as the doors are in the middle of closing, the monster jams it's massive claw through the shrinking gap and blocks it.
However, as the monster tries to bust the door open, it begins to convulses in agony. With it's claw still jammed into the door, a secondary head starts growing out of it's neck causing it to pull back momentarily. When the door finally slams shut with Sherry and Claire safely inside, the elevator begins to descend. The creature then lets out a loud roar and jumps onto the top of the elevator snapping the cable and causing the elevator it to fall down the shaft.
