On 30 October 2017, Mr. Webster was on a boat removing debris at Knostrop Weir when the boat capsised in turbulent waters. Despite efforts to save him, he tragically died on 1 November 2017.

A Health and Safety Executive (HSE) investigation revealed that BAM Nuttall Ltd had trained operatives who could have controlled the weir gates to slow the water flow, allowing debris to be safely removed. However, the company failed to execute this crucial task, leading to Mr. Webster's death.

BAM Nuttall Ltd pleaded guilty to breaching Section 2(1) of the Health and Safety at Work etc. Act 1974 and was fined at Leeds Magistrates’ Court on 12 June 2024.
