Tbh nah, under certain circumstances. For one we don't know the extent of the bullying, we also don't know if she couldn't find another donor kidney, and we don't know if the relationship or the bullying ever stopped.
There's too much being left out which is why I say he's NTA unless we learn some other things. For instance if they're still together or if the bullying stopped, if the sister ever felt remorseful or not.

From the information we got, yes you are. Yes she dated your highschool bully, and that was the main cause of your self-esteem, but would you really put some else's life in danger over that? Let alone your sister. I have experienced extreme bullying before, and i have never once that to take a human life over it. self-esteem is something you xan recover from, but risking your sister's life can be something you cant recover from (like regret, guilt, despair,or go full on into your evil arc).. (i dont know the full story, like if the sister joined in on the bully or how extreme the bullying really got or if it was minor, or the relationship between you and the sister before. but yes you are being a bit sensitive.)
