Thing is nihilus never actually died his body was dead yes but after his defeat in the original writings before it was toned down for KOTOR 2 nihilus just faded into dormancy so he can very much come back and it would be devastating if he did as theres no force wound present who could stop him so hed basically be unstoppable n just remember he achieved immortality when he bound his spirit to the robes and vitally the mask

I guarantee you this is Darth Nihilus's way of making the next Sith of Hunger. If the person was able to learn force drain and fed like Nihilus, the mask would have less of a negative effect on the wearer.
Not to mention they would be ridiculously powerful.
