It's definitely a joke because otherwise that would imply that Nolan & Annisa's grandparents are siblings.
Also, if this is true, Mark wouldn't be Annisa's 3rd cousin, she'd be his 2nd cousin, once removed.

I think it was just a joke insult. There's no way for Cecil knowing/confirming something like that (knowing Anissa is Omniman's Second-Cousin).
The 4 month break reference is just some subtle Meta-Humor. To us the audience, Cecil is referring to the actual 4 month break mid-season; in-world he's referring to when Invincible went off on to that mission to help the Thraxans. Invincible has moments of Meta Humor. Even during Season 2 (and in the comics), we had that moment in the convention where Mark's most famous animator is telling him a bunch of animation techniques & tricks - the show showing us examples while they're talking.

I feel like that 2nd cousin was more of an insult directed towards her, cause that "Some 2nd Cousin" sounded less like she was a 2nd cousin, but moreso a very off hand insult
