What first comes to mine is the shooter shot a squib load. That’s shooting a cartridge with an insufficient powder charge to propel the bullet out of the barrel and thus when the shooter loads another round into the chamber and fires again the bullet will greet the earlier fired bullet in the barrel which forces propellant gas to seek a less resistant escape path. The new path is what causes the rifle to explode.

Either an incorrect load for the barrel, or there was a squib load which left the barrel blocked. If you squeeze the trigger and it goes pop and not bang, DO NOT FIRE AGAIN, drop the mag, clear the chamber, and check the barrel with a cleaning rod! All while keeping the barrel pointed in a safe (downrange) direction of course!

One of 2 things happened here. Got mags mixed up and loaded a 300blk in a 5.56 rifle.
Or the more common, the previous round fired was what is referred to as a “pop and no kick.” Which caused the previous bullet to become lodged in the barrel caused by an under powered load. The rifle likely didn’t cycle properly and they thought it was just a malfunction. So they manually cycled the next round into battery and fired. With the barrel being clogged the gas had no way to escape and caused the upper to explode. Seen it happen in person before. Played out almost identical.

About 15 yrs. ago I had an AR malfunction, I had a round that felt and sounded very weak, but it did cycle the next round , and completely ignoring any common sense, I squeezed the trigger again and had a major malfunction similar to this guys, I'm assuming the previous round in my rifle never cleared the barrel, that was totally on me, I've always kicked myself for not taking that small amount of time to just double check things, maybe my admission of absolute stupidity can help someone else from making the same mistake, I got very lucky and only received a few very minor cuts on top of my right hand, the potential worst was something hitting my safety glasses and leaving a small scratch just about where my right eye ball was, be safe all..

For those saying a 300blk round can't "chamber"/"fire" in a .223/5.56 barrel, IT DEFINITELY CAN !!!
A 300blk round is essentially a shortened .223 casing, then necked up to fit a .308 diameter bullet. If you were to drop a SPENT 300blk casing into a .223 chamber, it would easily fit.... so here is what happens. When a complete/live 300blk round is tried to be chambered into a .223 barrel, the bullet COULD get pushed back into the casing when it hits the feed ramp and/or throat, thereby now causing it to be short enough to fit/chamber/fired. When fired, the bullet will get stuck in the throat, then gets over pressured, then goes 💥.... so, whenever there are .223 AND 300blk rounds in close proximity of each other, make sure all shooters are aware of it so they don't get mixed up
