The reason Genya looks like this is because Genya (Probably and you 100% know that) Genya feeds on demons and gains strength temporarily. In Infinity fortess, he helped the Hashiras in the battle of Kokushibo vs Sanemi + Gyomei + Muichiro. Genya ate an amount of Kokushibo's hair that had fallen during the battle. And it gained some strength. His eyes had changed into those of Kokushibo, and he looked a bit like Kokushibo. But Kokushibo cut Genya in half so that he wouldn't get in the way during the battle. And Genya died. His last words before he died were: "Thank you....-Ne...-mi...." Sanemi was very saddened by his brother's death. And during the war, he unlocked the demon slayer mark. He was able to finish the life-or-death battle against Muzan without a mortal blow, but died at the age of 25 for unlocking the mark.
