The Lord High Admiral is an appointment (nor a rank) that still exists (its usually held in commission or by the monarch personally though)-the last holder other than the monarch was the late Duke of Edinburgh.
The First Sea Lord was originally just the most senior of the guys exercising the office of Lord High Admiral while it was in commission, with the Second Sea Lord being the next one (just like the Prime Minister is technically also the First Lord of the Treasury, and the Chancellor of the Exchequer is the second Lord of the Treasury, they exercising the office of Lord High Treasurer), and until the Council of Admiralty was merged with its Army and RAF counterparts to form the Defence Council; the First Lord of the Admiralty was the civilian member.

America has a long tradition of not giving people high military ranks, we didn't have a second lieutenant general until Grant and when Pershing was made a full general a law was passed that automatically postumistly awards George Washington with a new rank as soon as someone attaines his current rank. So legally Washington is a 6 star general.

In 1857, the US established the rank of Flag Officer, which was basically the same thing. Prior to that, the highest rank was Captain, and squadrons were commanded by senior captains holding the positional courtesy title of Commodore.
