Compartment syndrome describes a dangerous accumulation of fluid within a muscular compartment of a limb that can lead to impaired blood flow, decreased oxygen delivery and cell death, in severe cases. This condition is classified as either acute or chronic, with acute often being a medical emergency that requires surgical intervention.

In this video, my friend @evan_ask shows how his leg appears now after suffering an acute compartment syndrome of the quadriceps compartment several years ago. He experienced this injury after receiving a hard check during a hockey game and immediately noticed swelling and sensory and motor changes in his leg. He was rushed to the emergency department of the local hospital where they splayed his thigh open, cutting some of the fascia, which allowed the pressure to drop and prevent any permanent damage to the femoral nerve. This procedure is known as a fasciotomy. His leg was left open for several days to allow intracompartmental pressure to decrease. During this time, blood clots were removed daily by hand and the wound was closely monitored for signs of infection.

Following surgery, rehab focused mainly on regaining knee flexion range of motion. He reports that strength came back without problem, but still has numbness around the incision.

These days, he has no pain or issues with function and competes professionally in motorcycle racing. Pretty cool case!

Was playing basketball when a kid kneed me in the thigh and burst an artery. Had a hematoma + compartment syndrome. Was geeked off morphine and percocet for the first week as I had a double surgery. Just discharged today. Everything hurts omg, trying to stay off the clouds now but literally moving my quad in anyway hurts. If anyone has advice please let me know. Will update this comment letting y'all know how I'm doing. Feel hopeless rn like it's never going to heal.

Brother - I got the same thing but on both thighs and both lower legs. Had to open me up on both sides of both legs - from my ass to my ankle. Glad to see you walking and on your feet man.

I ignored the symptoms I had during compartment syndrome. As anyone who’s ever had compartment syndrome you know how painful this is. I had no trauma to my leg. I just woke up one morning to some pain and swelling. As the day wore on I was no longer able to walk. This was at the height of the lockdowns when people were advised not to go to the ER unless you knew for sure it was an emergency. As I really didn’t see how a swollen painful leg was an emergency I decided to deal with it. As you’d understand the pain did get to be too much. By the time I got to the ER I was begging for pain medicine. I thought they were going to diagnose a blood clot. But no. It was compartment syndrome. I was at a small hospital so they weren’t sure if they were going to transfer me or have a special surgeon come in. They went with the latter. I will say that they did (thankfully) pump me so full of pain medicine that much of that night was a blur. Just a veiled recollection of talking to the surgeon before the procedure and the only reason I can barely remember is because of the fear when I learned I could lose my leg. Thankfully I did not lose it. But I did lose 3/4 of muscles on the shin due to necrosis. Two weeks in hospital and a surgery every other day. Ended up with foot drop syndrome and another couple of surgeries after the hospital. Skin graft, a monster scar on the right side of my leg from my knee to ankle. A 6 inch scar on the left side of my leg where they took muscle. Months of physical therapy. The nerve pain I still endure at times is the most horrible pain I’ve ever endured.
Long story short, don’t ignore signs your body is giving you. Even if you think it may be nothing, it could be everything.

I had broken my foot in four places (including two crush injuries) and ended up getting compartment syndrome. They took me in for emergency surgery and said if I didn’t say something, I probably would’ve lost my foot. What was supposed to be a 10min surgery ended up being a 2 hr long one because of the bleeding. After the surgery my foot felt 99% better. Four days later I had the fracture surgery and they stitched it up. It is no joke and neither is the test for it either 😬
