If he is hearing voices, this probably means he has schizophrenia. this mental illness is exactly of jimenez said, he was hearing voices in his head, telling him to kill those two teens to save the world. A similar scenario could be like a person, and random voices in their head telling them to run around naked, or kill particular people, otherwise there would be dire consequences, such as, that non-existent voice killing their family, etc. It is a serious condition, and even tho this guy is an absolute monster, there could be a reason for this action.
