I am a Controls Engineer, and the test lab I work at had machines which needed access doors to be opened while it was running in order to take measurements. I installed a bypass keyswitch to allow the machine to go into "inspection mode," which allowed those doors and ONLY those doors to be opened without stopping the machine. I also chose to use 'captive key' keyswitches, so the operator's bypass key was stuck in the machine until they turned off the bypass. I also adjusted the machine so that the tower light over the machine would display an attention-getting flashing pattern so that a bypassed machine could be identified from anywhere in the test room.

I helped our Bakery at WalMart and the bread slicer always made be uncomfortable, same with the Deli meat/cheese slicers. There isn't any protection besides best practice. For the bread slicers the practice was to have the weight of the bread push it through the machine, which many people didn't do because one loaf would take too long to go through. There would normally be a weighted plate that help push the bread so you never pushed the bread by hand but ours was missing and people just pushed the bread into the blades. Three loaves was the magic spot, just enough weight to push the first loaf through and have enough time to bag before the second was finished. I personally just used another loaf as a stick because I didn't want to get cut. The Deli slicers I never touched and refused. Almost every week someone cut themselves and a few people had to go to the hospital. If your attention slipped for one moment or if you used too much force your hand went with the product right into the blade. Newer machines could guide the product for you but not ours.

We had an entire course about Production lines, and one part of it was 2 lectures purely about all the things workers do to bypass safety mechanisms and how to spot it. It is INSANE how creative people get to save 2 seconds, but not their limbs... The lectures where called Engineers vs. Workers.

$3M was the available insurance in the case. The seller had gone out of business between when they sold the slicer and when my client was hurt.
