Medical error also could be under reported a lot more in the US because apparently it is a common deal between hospitals and surviving family members to waive all medical costs as long as you waive the right to sue, and subsequently they probably don't report it as medical error.

I am glad that you ended on the factoid that you did as one of the first things I was wondering is how many of the doctor caused deaths happened on people that were not going to be leaving the hospital in the first place, i.e. what you ended with. Seems like that would be a pretty high number. With age, disease, the ER taking in car wreck victims, gunshot victims, etc. errors could be made on someone...that would die regardless.
