The cow is getting its medicine. The reason why it's being inserted like that, is because otherwise the cow would have just spit or vomited the medication out. It's the same thing with cats and dogs at the vet. Also, it doesn't hurt them.
Humans have the capacity to understand why they have to take their medication(s), while on the other hand animals don't.

(My initial comment was innacurate in a few places). Basically, using the rod is the best technique to insert the magnet, because it doesn't hurt the cow and it's the most efficient. And it's different for smaller domestic animals like cats or dogs. It doesn't hurt the cow, it's just uncomfortable, especially the pulling on the nose. *(If you want more details then just read the below)*.]
Further talk: (I'm not here to talk about how killing animals is bad and so forth, just about this video. If you want to know, I'm not a complete vegan but I don't agree with killing animals for food, especially in the 21st century).
I got some things wrong in my initial comment, and didn't expect it to get so many likes.
First of all, the medicine is actually a magnet that needs to be inserted in a single dose, and can't be cut up and mixed with food, because the cow might not like to eat it by itself.
Secondly, cows can't vomit, they can re-chew their food, and it depends on the cow and environment if the cow takes its medicine with the food, but in this case the magnet the cow is taking is most effectively and efficiently inserted with a rod. And just to clarify, the rod had no right to hurt the cow, and the only thing I see that could hurt it would be the pulling on the nostrils. It's also the case of if cows feel pain differently from humans, but there is no way currently to prove that.
Thirdly, I was wrong in the way I said "it's the same with cats and dogs." It isn't. Honestly idk what I was getting at there, but just to clarify, it depends on the medicine and the animal. In this case it was a cow and had to be inserted this way.
Lastly, the part about the capacity of understanding why you have to take your medications.
If you train an animal or make it understand in some way or another why and to take its medicine, then that's great. If it's a cow on a farm, you could train it to take its medicine, but it also depends on the farm itself. If it isn't efficient to train every cow to take its medicine, because of size related issues, then I understand why it doesn't happen. But it also depends on the animals capacity to TAKE IN the medication. Some would have difficulty in taking or swallowing it and/or others could accidentally chew on it (like a magnet in this video).
The technique shown in the video is most efficient, and shouldn't hurt the cow in any major way.
Now, I'm not going to get into any moral issues of veganism/vegetarianism and preserving the lives of animals. This is only refering to the cow's intake of medicine, nothing more. It isn't pushing an agenda or anything of that kind.

This is necessary in feeding livestock medicine specifically cows. You kinda have to forcefully shove medicine or antibiotics down their throat only because cows graze and chew for hours on end then swallow. If you gave them the pill they would chew it for hours. Easier to do just that shoving it through. Enjoy your milk and cheese and your beef π
