As a tattoo artist, I have declined people for being too drunk or too messed up to get a tattoo. I would do the same for medical surgery too.

First off I wouldn’t have operated in the first place as she didn’t have enough respect for herself or me to follow pre-op directions. So, therefore I wouldn’t have done a thing.I would also tell her she needs to find another plastic surgeon.

I was required to sign a contract with my surgeon several months before my gastric bypass. I had been quit 2 years, so no troubles. In that contract, you agree to testing. If you test positive for nicotine, your surgery is canceled.

I'd say watch and wait. Even though she may deny the use of tobacco you do what she agreed to. Post surgery you advise her about the outcome and possibility of losing the nipple asking again about smoking. The chance of the nipple surviving isn't that good so you tell her what to watch for and to call you if there's no improvement. She didn't agree to anything else so unless you get her permission post surgery to use the leeches, you can't take it upon yourself to make the decision for her. Consent is key to any procedure. Remember malpractice lawsuits are a very real possibility so you need to protect yourself and refrain from going outside the agreed consent

When I had my top surgery I was told not to smoke leading up to it because of the grafts. My nipples were removed and reshaped and put back on and thankfully they attached beautifully :)
Personally.. idk, lol, I would have imagined that the surgery would have been cancelled to begin with. Considering how important it is for good blood flow the risks wouldn’t be worth it.

I would have questioned the patient throughly before the surgery and would have postponed it or canceled if she had not followed instructions to quit smoking. This is too important for her to ignore and she must be made aware.

I wouldn't have done the surgery because she didn't follow instructions & it would have been way too much of a risk. However, once here, more surgery with her poor circulation doesn't seem sound & the leeches could possibly provide that which is necessary while simultaneously being non-invasive. Is that a good answer?

Ask if she smoked. Her reaction to the question would giver her away. That being said, get the leeches.
