Caution is always best, maybe the pedestrian lights are out or out of sync, definitely not a terrible error but always check both.

If one of the lights isn't working who's to say the pedestrians don't have a green light too he was right to be cautious

An aggressive driver would say the green light is working and a defensive safe driver would go when both turn green. He was right to not trust this scenario its a no win situation. He could go when the right one is green and then get pulled for why he didn't wait? That's just cruel to fail him for being a safe defensive driver especially as he's learning. Atleast you know his decisions are making roads safer which in reality should make an instructor happy. What kind of drivers do you want on the road? I'd rather have him on the road than aggressive drivers looking for any opportunity to go which could put my family at risk.
