The books go into detail for Johnson.
He also survives the flood because he is one of the extremely rare Spartan 1 survivors. His nervous system is all screwy and the flood can't latch onto it.

He only canonically died once and that was during Halo 3. spoilers for the First Strike book Johnson is immune (technically just highly resistant if I remember right) to the flood because of the Spartan 1 program. He fought his way out. After Chief and Cortana make it off of the Halo they find Johnson and some more survivors and link up with them. They take over a covenant carrier and jump to Reach. At that point they found some Spartans, rescued Halsey and blew up a Covenant fleet before going back to Earth.
