Actually, it was always Tiberiusā¦. An often overlooked plot point in āWhere no man has gone beforeā that Kirk felt justified in killing Gary Mitchell because that Gary misspelled his supposedly āgood friendāsā name on his tombstone.

According to one of the novels, Kirk and Mitchell used to have a joke that Kirk's middle name was "racquetball". This was used as a retcon and to show that evil Mitchell had a morbid sense of humor.
