Yesterday night, Feb 28th to March 1st, a cargo train and a passenger train connecting the capital city with the second biggest city of the country, collided head-on in Central Greece, as if lived in the 1950s. There were about 60 dead and still counting and still gathering hands and legs, in what is the biggest railway accident in Greek history. Most or them were university students and soldiers on their compulsory military service.
Telematics equipment for our railroads was purchased in 2000, but nobody has taken the trouble to install the equipment as of today, and it is still waiting in storage. Three layers of other collision prevention technology were all out of service simultaneously, leaving all communications between station masters and drivers taking place with the radio, and verbally describing one another roughly were they are, using landmarks.
An inexperienced station master in the halfway large city of Larissa made a moment's mistake. The rail workers union had sent multiple warnings to the companies and the government that we can't go on telling one another "We are now passing by that tree or that valley" through radio, but they were given humiliating responses. They had also gone on strike, but they were presented on TV then as "political extremists who like to trouble commuters".
The state-owned company co-ordinating movement is guilty for the railroad systems inoperation, the lack of equipment and crm, and the private company owning the trains themselves is guilty for pressing drivers not to delay, the systems inoperation on the trains themselves and throwing too fast trains on the network before it is fixed. The owner of the train company is Italy's state owned train company, acting as a private owner of the train company here in Greece. The safety of hundrends or thousands of people every day is totally dependent on the station master with the beret on the head and the whistle around the neck.
Relatives, friends and partners of the young deceased gathered and demonstrated peacefully with candles yesterday afternoon, only to be met with horrific police violence. Now, the government, the train owners and the media all try to pin the whole of it on the station master and conduct all public rage towards him, who made the momentary mistake.

One of the things that was missing in my PPL training was practicing having problems with checklist items. You get so used to reading and confirming that it’s not hard to actually have that as muscle memory and completely miss that something you just confirmed was ok is in fact not ok.
More so with single pilot ops, but can happen with two pilots too.

I was aircrew on SAR helos in the USCG. Some checklists never leave you; I could still start the APU and engines of an HH-3f if, for some odd reason, I had to.
Years ago, I managed to get my wife to use elements from the Before Taxi checklist from the HH-3f when we drive together. She does the responses in the passenger seat and calls when she drives. She even chides me if I forget to respond properly!
