I think it's a combination of both. After all people get into the backrooms via a glitch in reality so there may be spots where they boarder the frontrooms. At the same time, with so many levels that drain or damage sanity on top of the despair, anxiety and loneliness and the stress found in Many levels its also completely common for people to have a mental break and hear or see shit that ain't there and think they're seeing or hearing the frontrooms

I personally think people in the backrooms are just going insane. Transferring to a life of survival rather than your current one can be difficult, so they've gone insane for such a drastic change.

Its probably a mix of both hypothesis, cus we already know a few levels and entities that mimic things in the frontrooms, but also sanity is a big thing in the backrooms, so yea it could be both

In my canon, this is a sign that you are nearing an EXIT to the backrooms. This is incredibly bad. It is recommended that you flee the area and never look back.
