The population of Denmark is close to 6 million people. Now, the breakdown of demographics on people who need and use healthcare is a small fraction. College is a small fraction. Taxes are high there, but their taxes are used wisely by the government. The 5 Burroughs of New York is almost 8.9 million people. I only point this out because it is needed to get the correct perspective of population sizes when comparing countries social programs.
