As a land surveyor, I see something similar all the time. When we open storm system manholes or grates, they will often be recessed away from the actual opening, and if it is deep enough to require a ladder by law, about 90% that I’ve seen are on the wrong wall. How are you supposed to climb down if you can’t even touch the ladder?
