Rachets with release buttons can be pressed down while turning the handle , gives you the friction to turn the rachet.

Or just hold the extension in place so it doesn’t spin both ways then the ratchet work as intended. And if you reply saying what if there isn’t room for your other hand. Well if there’s room for you to hold the top of the extension there’s room for you to stop it spinning…

I’ve dealt with this too many times to count. Never had this issue. I just wiped my hands and the ratchet and hold the bit or extension with my other hand.

Couldn't you just grab the extension/All the bits beneath that? Then when you go back for the ratchet to do it's thing it works and you let go when it goes forward to tighten the bolt/screw
