I think that a pain block from a skilled anesthesiologist is one of the greatest gifts that a patient could ever receive! As a PACU RN, I helped our anesthesiologists place hundreds of blocks,(pre and post op) and witnessed the absolute miracle that happens for that patient. I have also seen what the pain is like for patients who did not have that option and the great difficulty we had trying to manage the high level of pain associated with those kinds of surgeries.
As a patient who underwent a torn shoulder rotator cuff repair and had a supraclavicular pain block, I can tell you that I would never recommend undergoing this surgery without it! I was healed both by my surgeon and by my anesthesiologist.
Even with the broken health care system that we struggle with in our country, we are incredibly blessed and I am profoundly grateful.

Ive noticed this too. It brings it down to a manageable level and then regulates it at that level

My husband had a rotator cuff repair, and he had Exparel for a nerve block. The block lasted almost 4 days. By the time the block wore off, he really didn’t have any pain. Did make sure that he had taken some oxycodone prior to the block wearing off, but he turned out not to need it.
That said, from what I have read in the literature Exparel doesn’t always work.
Everybody is different.

I had the same operation on my ankle twice. I can say that the second one with the nerve block was a much easier recovery. I was able to get ahead of the pain. I didn't wait to take my pain meds. I took them as advised by my ortho. I also didn't need as much sedation and was able to leave hospital sooner on second one. The first one without the nerve block was a TERRIBLE experience!

The first time I had a nerve block they had given me versed before they did it which helped tremendously. The second time I had one I didn’t get anything prior to the procedure and it was one of the most awful experiences I have ever had.

Why can’t people come back for another nerve block to not have the pain for a few days? When I had shoulder surgery I would have loved to come back for another one. By the way I watch you all the time! I wish I could have you for my anesthesiologist.

I’m all for nerve blocks for a temporary surgery situation however “injections” for extensive chronic pain not effective at least for me in the on-going battle to manage such severe chronic pain. I’m always amazed to think that a person who follows the rules, who has been carefully managed on a stable dose of opioids is treated like a person with a contagious disease. I’m still scared due to doctor retirement and hoping to find a solution. I’m willing to seek a consultation even though it will require travel. Regardless thank you for all you do to help people and educate people regarding the current medical landscape.

You aren’t kidding, though it was much more painful after knee replacement than hysterectomy.

I felt the entire procedure after the block wore off. Like an actual replay of the surgery step by step

I had a nerve block for my knee surgery and nobody warned me about the potential for extreme muscle spasms/cramps, so I ended up with an unnecessary, expensive ER bill the night after the surgery. the way I've described the pain ever since is like labor pains but limited to specifically the muscles surrounding my knee cap. the surgery was a mixture of arthroscopy to release a ligament and cutting part of the tibia to tighten tendons for repeated patellar dislocations, if that gives reference to what muscles I'm talking about. it felt like my knee cap was dislocating itself over and over and over, but when looking at it, that wasn't the case. I've had a lot of pain in my life but that is still the worst I've ever experienced in my chronically ill 21 years of life.

the nerve block worked really well for me and didn't wear off before it was supposed to, which was kind of surprising to me because my body has a history of metabolizing medications quicker than the average person. I've never had a problem with general anesthesia as far as I know, so maybe those drugs and nerve blocks don't have the same problem with metabolizing too quickly for me.

You are great doc. Just wish the professionals would make sure to adequately control post op agony and not have the mindset of making it tolerable. Cheers all.

A few months ago, received a nerve block, prior to shoulder surgery. Yes, for about 24 hours, there was no pain. But, as that wore off, a level 10 pain set in. I was given an rx for hydrocodone and it took the edge off. I’ve had quite a few surgeries so knew to expect pain. But, this one was far worse than expected . I really don’t think the block accomplished anything , beyond the 24 hours. I’m sure everyone has a different experience though.
