In a third scenario with same presentation:
If patient has harsh holosystolic murmur with palpable thrill at left sternal border then it is rupture of inter-ventricular septum.
In mitral regurgitation, palpable thrill is absent and holosystolic murmur is soft
In ventricular free wall rupture, murmur is absent due to cardiac tanponade
