a nilbog happens when a goblin gets abused a bit to much so goblins will set aside a single goblin to do what ever it wants and no one can stop it in hopes that will keep the nilbog from possessing a goblin or something like that

I've used a Nilbog as a low level boss before.
Was a truly fun encounter too.
Scared the 11 year old player at the table with the very much practiced laugh.
That was a good night.

fun fact, if you use the 6th lvl spell to summon one, and yo ucast pro evil on yourself and slap it for 10minutes straight you will have a 1000hp nilbog for the next 50 minutes. just give it at trash +1 weapon and let it go Ham on the people.
