Community Healthcare Network (CHN) is a network of 11 federally qualified health centers that provide quality healthcare to underserved communities in New York City. CHN launched an agency-wide health literacy initiative in 2011. CHN’s health literacy initiative aims to integrate health literacy into every aspect of organizational culture to improve patient outcomes.
