Diabetes is one of the biggest challenges facing the NHS. Estimates suggest that 8.3% of people in Liverpool may develop diabetes by 2030 unless more support is provided to help people take better care of their health and well-being.
Liverpool Diabetes Partnership became fully operation in 2015 and designed to tackle this challenge head-on, with a shared vision to improve outcomes for people living with diabetes. It forms part of the Healthy Liverpool Programme, which aims to transform healthcare in the city so that everyone receives âconsistent, high-quality healthcareâ that will lead to âlonger, healthier livesâ.
